GRANT APPLICATION PROCEDURE

Each proposal MUST include the following items, in order:

     1.       APPLICANT INFORMATION

                             Title of Project

           Name(s) of Applicants

                             Building(s)

                             Job Title(s)

                             Email Address(es)

                             Number of Students Impacted

2.       FOCUS OF GRANT

In one or two sentences, state the focus of your grant proposal.

3.       RATIONALE

Explain the need/rationale for this proposal.  What is the problem that you are seeking to address, the skill(s) that you are trying to improve, or the other school adjustment factor that you are targeting (e.g., motivation, engagement, achievement)?

4.       COMPATIBILITY WITH 2009-2014 STRATEGIC PLAN

Applications should describe how the proposed project is related to and/or is designed to support at least one of Homewood School District 153's 2009-2014 Strategic Plan priorities and goals as stated in the Focus Areas.

5        DESCRIPTION

Describe the proposal thoroughly.

6.       PROPOSED TIME FRAME

Provide and estimated start and completion date for your grant request.

7.       STAFF INVOLVEMENT

State what personnel will be involved and how. Include all staff members who will be involved in executing this grant who are not named as the applicants of this proposal.  If you anticipate the need for district personnel for technical support include that information as well.

8.       EVALUATION

Describe how the project will be evaluated for its effectiveness. Be specific as to the methods used and the criteria that will indicate development, achievement, engagement, or other educational/developmental measures.  All funded applicants will be expected to complete a Project Evaluation Form within 30 days of the completion of the project, but no later than the last day of regular classes for the school year.

9.       BUDGET

Provide a budget, with detailed explanation.  Budget components will include as appropriate: staff time during the summer or outside the school day, release time (limited), use of resource personnel both in and outside the district, travel (staff and student), site visits, workshop fees, supplies.  Hardware and equipment normally secured through district funds will not be supported by the grant.  Any hardware, equipment or supplies purchased with grant funds become the property of the district.

Grant limits:

Grants to an individual or one classroom will not exceed $750.

Multiple classrooms or building grants will not exceed $1,750.

If your project budget exceeds the grant limits, provide information for how the balance of the funds will be obtained.

10.    SUPPORT MATERIALS

Support materials may be attached to the application. Support material can be submitted in the form of links to relevant websites.

11.    INFORM THE BUILDING PRINCIPAL

Applicants are expected to apprise their building principal of the submission of an application and, if awarded, the progress of its proposed activities.
